Eligibility Requirements:

1. Must have been a resident of Ashland County for the year preceding admission, if not a self paying resident.
2. Must be admitted under the care of a physician.
3. Must have a diagnosis for which we are staffed, and able to provide for.
4. Must not have a communicable disease or active tuberculosis.
5. Must not have a moderate to severe behavior problem.
6. Must be dependent on the services of others because of age or physical, social or mental impairments.
7. May need assistance in the daily activities of living, but not be totally dependent for lifting or transferring.
8. Must not have I.V.'s, N.G. tubes, or tube feedings.
9. Must not need extensive treatments requiring skilled care plans, for more than a temporary basis.
